Requests for records to be sent out from

Pediatric Medical Care  will be complete with all possible expediency.

​

Families should allow 7 days for forms and up to 30 days for complete record requests.

 

Medical records will be provided in digital form or may be faxed at no cost. Requests for printed copies of records may incur charges which reflect out printing costs.
